Comparison Of Laparoscopy Versus Laparotomy In The Management Of Adnexal Torsion

The aim of this study was to compare laparoscopy to laparotomy in the management of patients with suspected adnexal torsion. Seventy two patients with initial diagnosis of adnexal torsion that presented to our clinic with acute abdomen or were referred from other centers between January 2007 and December 2009 were included in this retrospective analysis. Data regarding age, gravidity, parity, size of adnexal mass, pre-operative and post-operative hemoglobin values, operation time and duration of hospitalization of patients that had adnexal detorsion, cystectomy, oopherectomy, total abdominal hysterectomy and bilateral salpingo-oopherectomy (TAH-BSO) performed via laparoscopy or laparotomy were recorded. Twenty eight patients were treated laparoscopically while 44 patients had laparotomy. In the laparoscopy group 6 cases had detorsion, 17 had detorsion and cystectomy and 5 cases had salpingo-oopherectomy. In the laparotomy group 3 had detorsion, 17 had detorsion and cystectomy, 7 had salpingo-oopherectomy and 17 patients had TAHBSO. Laparoscopy group consisted of young patients with low parity, operation time was shorter in laparoscopy group and the patients were discharged more rapidly. Average adnexal mass size was bigger in laparotomy group. Adnexal torsion is one of gynecological emergency that require urgent operation the most appropriate surgical approach is laparoscopy especially in patients who want to preserve their fertility.
