Extrapleural Abscess In The Posterior Mediasten In The Diabetic Patient

57 years old male patient who admitted our clinic from the intensive care unit because of the one week refracter of the treatment pleural effusion with the septic situation. The patient is a diabetic (type 2) for 13 years and lastly two years use insulin and last 10 days he has dispne, right costal pain, fever and don’t regulate blood glucose profile. The patients tension arteriale was 85/60 mmHg, heart rate was 120/min; leucocytosis 23000 and blood glucose level 470 in laboratory. PA chest film and computed thoracic tomography were reported lung abscess which the right posterior mediastineum and extrapleural look the air-fluid level. As a result of thoracentesis pleural empyema in a patient with pure liquid under the most comfortable coming right scapula chest tube (no 28) was inserted 1700 cc of purulent fluid was taking and in this case was achieved dramatic clinical improvement. Blood sugar was regulated daily pouch was washed with 300 cc riphosine fluid 10 days later, tube made under a pet. The situation is completely stable at the end of the first month.This is a rare case report of lung abscess, and being seen in the posterior mediastinum was only due to be treated with chest tube
