Fever Of Unknown Origin, With Mtfhr-C677t Gene Polymorphisms And Portal Vein Thrombus

Fever of unknown origin is defined as the following a temperature greater than 38.3°C (101°F) on several occasions, more than 3 weeks’ duration of illness, and failure to reach a diagnosis despite 1 week of inpatient investigation. Evaluation of patients with FUO is complicated because there are multiple etiologic factors and new ones are adding in literature every day. A 24years old man was performed contrast enhanced CT for FUO and abdominal pain, show trombus in portal vein and superior mezenteric vein. On examination, metiltetrahidrofolate reductase gene polimorphism is detected in the etiology of trombus. The patient’s fever and abdominal pain improved after 5 day treatment of Clexane 0,8 ml. A further doppler sonography show partial regression in trombus by which time the patient’s symptoms resolved. We aimed to present clinical and radiologic imaging findings in a case of portal and superior mesenteric vein trombus associated with MTFHR-C677T gene polimorphism which is a rare cause of FUO and we didn’t meet in the literature.
