Mechanical ventilation (MV) is the most important part of contemporary critical care concept and one of the most frequent reasons for intensive care admissions. Currently, there is a tendency to use the mechanical ventilators in the intensive care units by pediatric intensivists. The aim of this review is to summarize the basic principles of mechanical ventilation in order to be applied when required by all physicians. There is an indication for MV when the spontaneous respiration is threatened to sustain life. Currently, the principal criteria to determine the need for MV is the clinical judgement of the physician rather than laboratory results of the patients. Some of terminology should be well- known to initiate the MV. The main difference between the basic ventilator modes ventilator’s ability to detect and and respond to patient’s own spontaneous respiration.
