Angina Pectoris in Aortic Stenosis and Relation To Coronary Artery Disease

In aortic stenosis, in order to evaluate angina pectoris and association with coronary artery disease (CAD), and to determine the necessity of performing coronary arteriography, 173 patients who were performed coronary arteriography between 1978 and 1990 (153 men and 20 women, mean age 50.6±9.2, range 31-76) were analysed retrospectively. 149 of them (86%) had angina pecwris. With increasing aortic gradient (below 50 mmHg, between 50-100, above 100), the prevalance of angina pectoris increased (82% , 85%, 88.5% respectively) (P>0 .05 ). 46 patients had CAD (26.5%). Of those with angina pectoris 42 (28%) had CAD, while 4 (%17) of 24 patients without angina pectoris had CAD (P>0.05). With increasing aortic gradient (below 50 mmHg, between 50-100, above 100) the prevalance of CAD decreased (41%, 20%, 8%) respectively). It was concluded that coronary arteriography should be performed in all patients over the age of 40 years with or without angina pectoris and the higher the aortic gradient, the lesser the coronary arteriography will be necessary.
