Unicornuate uterus with rudimentary horn is the rarest congenital anatomic anomaly of women genital system and usually develops following insufficient development of Mullerian ducts causing many life threatening obstetrical and gynecologic complications unless diagnosed and managed properly. We present a 31 years old patient applied to our clinic with a his-tory of 4 months amenorrhea and progressively incerasing low abdominal pelvic pain. Abdominal sonographic examination revealed a nonviable fetus at 16 weeks of gestation, lying in a rudimentary horn. Elective laparotomy was performed and the unrupture rudimentary horn with a nonviable fetus was totally excised by laparatomy. Post-operative recovery was uneventful. The need for a high index of suspicion and the role of ultrasonography in the accurate diagnosis is highlighted.
