Aim: Epidermoid cyst is an uncommon ectoderm originated benign cyst. Although intracranial epidermoid cyst may commonly occur in the cerebellopontine angle and chiasma opticum, it can be detected in cerebral hemispheres as well as intraventricular area. Syringomyelia, a progressive hydrodynamic disorder of spinal canal usually occurs in cervical region although it can be seen in thoracic and lomber portion of the spinal cord. Case report: We describe a patient with syringomyelia associated with posterior fossa located epidermoid cyst. Siringomyelia has been spontaneously regressed after the removal of the posterior fossa epidermoid cyst. Conclusion: Primary intervention to spinal cord cavitations must be avoided because it can be spontaneously regressed after the removal of the posterior fossa tumor.
