Delusional disorder is a psychiatric diagnosis that one or more persistent and insistent nonbizarre delusional belief is dominant in clinical feature. The delusional disorder somatic subtype is called as monosymptomatic hypochondriacal psychosis. In this disorder one believes that he/she has a disease and has hallucinations pertinent to this delusional belief. In this report, a monosymptomatic hypochondriacal psychosis case which directed to psychiatry clinic because she presented to emergency service, internal medicine clinic and general surgery clinic of our hospital several times every day with stomachache due to the belief of presence of gastric ulcer, and insisted to be operated despite large number of examinations done were normal, was presented. It was shown that olanzapine is an effective treatment option that can be used in these cases. It is important to be taken into consideration this rare disease especially by non psychiatry clinicians both for early presentation of these patients to psychiatry clinic and to prevent unnecessary interventions by other clinicians.
