One of the rare infective factors of bacterial origin from the external ear canal is Achromobacter xylosoxidans. Although in this group of bacteria as the agent of infection in human rarely seen, this bacteria often encountered associated with water environments. Those most infections are nosocomial origin. Sources of infection include dialysis fluids, distilled or deionized water, tap water, chlorhexidine solutions and intravenous fluids. The disease usually affects patients due to other underlying disease, impaired immune resistance, using immunosuppressants, premature babies or organs impaired immune resistance due to trauma. Ear drops containing cortisone used for a long time in the presence of resistant external otitis external otitis due to Achromobacter xylosoxidans’a should be considered. Classical antibiotic treatment is ineffective. Trimethoprim-sulfamethoxazole, third-generation cephalosporins and antipseudomonal penicillins (piperacillin, ticarcillin, and carbapenems) ‘s should be used.
