Endoscopic retrograde cholangiopancreotography (ERCP) is commonly used for diagnosis and treatment in pancreatic and biliary diseases. Although it is a commonly used and safe procedure, it can also be related with several complications. These complications are generally mild, but few of of them may cause severe morbidity and mortality. We present a rare complication of ERCP with bilateral pneumothorax, extraluminal abdominal air, retroperitoneal and wide subcutaneous emphysema. The patient was transferred to intensive care unit (ICU). As a result; because of these rare but mortal complications, ERCP must be performed in departments which can be supported by intensive care units.
