ÖZ
Amaç: Diyabetik ayak ülserleri (DAÜ), diyabetin en ciddi ve maliyetli komplikasyonları arasında yer almakta olup, hastaların yaşam kalitesini önemli ölçüde azaltmakta ve hastaneye yatış ile amputasyon riskini artırmaktadır. Bu çalışma, DAÜ tanılı hastalarda ayak öz bakım davranışlarını ve diyabet öz-yönetim becerilerini değerlendirmeyi ve bu uygulamaların önemine yönelik farkındalık oluşturmayı amaçlamıştır. Gereçler ve Yöntem: Kesitsel ve gözlemsel nitelikteki bu çalışma, üçüncü basamak bir hastanenin Sualtı ve Hiperbarik Tıp ile Diyabetik Ayak Polikliniklerine başvuran 228 diyabetik ayak ülserli hasta ile yürütülmüştür. Katılımcıların demografik özellikleri, klinik verileri ve yara ile ilişkili değişkenleri kaydedilmiştir. Diyabet Öz-Yönetim Ölçeği (DÖYÖ) ve Diyabetik Ayak Öz Bakım Davranışları Ölçeği (DAÖBDÖ), yüz yüze görüşme yöntemiyle uygulanmıştır. İstatistiksel analizler SPSS v25.0 programı ile yapılmış ve anlamlılık düzeyi p<0.05 olarak kabul edilmiştir. Bulgular: Katılımcıların yaş ortalaması 63,1±10,5 yıl olup %72,4’ü kadındır. Hastaların %55,3’ü diyabetik ayak nedeniyle cerrahi geçirmiş, %38’i Wagner evre 4 yaraya sahiptir. Ortalama DÖYÖ puanı 6,6±1,7, medyan DAÖBDÖ puanı ise 28,0 (IQR: 9,0) olarak bulunmuştur. Eğitim düzeyi ile her iki ölçeğin alt boyut puanları arasında anlamlı pozitif ilişki saptanmıştır (p<0,001). Diyet kontrol puanı, cerrahi geçirenlerde ve Wagner evresi daha yüksek olanlarda daha yüksekti (sırasıyla p=0,019 ve p=0,003). Ancak, toplam DÖYÖ ve DAÖBDÖ puanları ile Wagner evresi arasında anlamlı bir korelasyon bulunmamıştır. Sonuç: Eğitim düzeyi yüksek olan bireylerde öz bakım ve öz-yönetim davranışları daha gelişmiş olup, komplikasyonların ciddiyeti de bu davranışları etkileyebilmektedir. Hedefe yönelik eğitim programları, hasta farkındalığını artırarak daha sağlıklı davranışları teşvik edebilir, DAÜ’ye bağlı komplikasyonları azaltabilir ve sağlık bakım maliyetlerini düşürebilir. Bu alanlarda yapılacak ileriye dönük çalışmalar, yaşam kalitesi üzerindeki doğrudan etkilerin daha iyi anlaşılmasını sağlayacaktır.
