Amaç: Diyabetik nefropati, kronik böbrek hastalığının ve son dönem böbrek yetmezliğine gidişin önde gelen nedenlerinden biridir. Nefropati şiddeti ile paralellik
gösteren yeni biyobelirteçlerin tanımlanması, risk sınıflandırması ve klinik karar verme süreçlerini geliştirebilir. Bu çalışmanın amacı yeni tanı almış diyabetes mellitus
hastalarında serum selenoprotein P düzeylerinin, böbrek fonksiyon parametreleri ve proteinüri şiddeti ile ilişkisini değerlendirmektir.
Gereç ve Yöntemler: Doksan diyabetes mellitus hastası proteinüri durumlarına göre üç gruba ayrıldı: proteinürisi olmayanlar (n=30), mikroalbüminürisi olanlar
(30–300 mg/g, n=30) ve makroalbüminürisi olanlar (>300 mg/g, n=30). Demografik özellikler, glisemik göstergeler, böbrek fonksiyon testleri ve serum selenoprotein
P konsantrasyonları değerlendirildi.
Bulgular: Selenoprotein P düzeyleri gruplar arasında kademeli olarak artış gösterdi (sırasıyla 1,61 [1,23-2,10], 3,28 [2,79-4,07] and, 7,13 [5,51-8,34], p <0,001).
Proteinüri şiddeti; selenoprotein P (r=0,794, p<0,001), kreatinin (r=0,532, p<0,001) ve HbA1c (r=0,406, p<0,001) ile güçlü pozitif korelasyon, GFR (r=−0,681, p<0,001)
ile ise negatif korelasyon sergiledi.
Sonuç: Artmış selenoprotein P konsantrasyonları, kötüleşen proteinüri ve böbrek fonksiyon bozukluğu ile yakından ilişkilidir ve diyabetik nefropatinin erken
biyobelirteci olma potansiyelini desteklemektedir.
