Şiddetli Mitral Yetersizliği Olan Hastalarda Plazma Kalprotektin Düzeyleri

ÖZ
Amaç:
Kalprotektin, fagositlerin sitoplazmasında bulunan ve inflamatuar yanıtta ekstrasellüler matrikse salınan bir proteindir. Önceki çalışmalar, kalp yetersizliği ve akut koroner sendromlu hastalarda plazma kalprotektin düzeylerinin yüksek olduğunu göstermiştir. Bu çalışmada, plazma kalprotektin düzeylerinin fonksiyonel ve dejeneratif mitral yetersizliği etiyolojilerine göre farklılık gösterip göstermediği araştırıldı.
Yöntem: Çalışmaya şiddetli fonksiyonel mitral yetersizliği (FMY) tanısı alan 24 hasta, şiddetli dejeneratif mitral yetersizliği (DMY) tanısı alan 36 hasta ve 22 sağlıklı kontrol dahil edildi. Tüm katılımcılara ekokardiyografi uygulandı. Plazma kalprotektin düzeyleri enzyme-linked immunosorbent assay (ELISA) yöntemiyle ölçüldü. Bulgular: Yaş, cinsiyet ve kardiyovasküler risk faktörleri açısından gruplar arasında anlamlı farklılık saptanmadı. Plazma kalprotektin düzeyleri FMY grubunda hem DMY grubuna hem de kontrol grubuna göre anlamlı derecede yüksek bulundu (sırasıyla p<0.001 ve p=0.002).
Sonuç: Bulgularımız, şiddetli FMY hastalarında plazma calprotectin düzeylerinin belirgin şekilde arttığını göstermektedir. Kalprotektin düzeyleri, mitral yetersizliğinden ziyade kalp yetersizliği ile daha fazla ilişkili görünmektedir.
