Achillestendonrupture is a commonmusculoskeletalinjury. Itusuallyoccursunilaterally in activemiddle-agedindividuals. Spontaneousruptureswithoutany risk factorspredisposingthepatient is uncommonandfor it tooccurbilaterally is veryuncommon. Wereport a case of bilateralspontaneousAchillestendonrupture in a 32- year-oldmanwithnocertainpreviousandcurrentrisk factors. Therupturesoccurredafterliftingbarbell in whichtheweightcaused a severe stressandelongation of thebilaterallygastrocnemiuscomplex.After a clinicalexaminationconfirmedthediagnosis, ultrasonographyof theAchillestendonsrevealedbilateralruptures. Thepatientunderwentbilateralprimarysurgicalrepair. Rehabilitationwasbegun 6weekslater. Weallowedtheweightbearingpartially at 8 weeksaftersurgery, fully at 12 weeksaftersurgery.Thepatient'sreturntopremorbidworkandsocial life wasuneventful.BilateralAchillestendonrupturesareveryrare but increasedawarenesswouldhelpavoid a rupture of thecontralateralsidebeingmissed.
