Internal carotid artery (ICA) punction is a major complication of percutaneous internal jugular venous canulation (IJVC). A 59 years old woman was scheduled to the right vena jugularis interna catheterization during the catheterization right carotid artery was punctured three time. After the last puncture suddenly left facial paralysis developed. On her examination, we realized the appearance of left hemiplegy and on the cranial CT there were subacute ischemic foci on the right fronto-parietal region. In the event of carotid artery punction during IVJC, excessive manual pressure should be avoided and the procedure should be better performed in the contrlateral side under USG guidance.
