Lipomas are the most common extratesticular scrotal neoplasms and they most often originate from the spermatic cord. The etiology is unknown but linked to a constitutional factors, obesity, an its presentation is more frequent in the sixth or fifth decade. In this case, an unusual massive scrotal lipoma that occured in 70 years old man is described.
