The İnvestigation of the Hemodynamic Changes in the Eye Before and After Trabeculectomy Operation in Primary Open Angle Glaucoma by Color Doppler Ultrasonography

Aim: The aim of this paper is to demonstrate hemodynamically the efficacy of glaucoma surgery in patients with primary open angle glaucoma and to elucidate the pathopysiology of glaucoma. Material and method: 45 eyes from 45 patients who were diagnosed with primary open angle glaucoma in Istanbul Goztepe Training and Investigation Hospital were included into the study . The inclusion criteria were the need for trabeculectomy and no previous eye operation. All patients have undergone trabeculectomy. In the examinations before and 15 and 60 days after the operation, visual acuity evaluation, refraction examination, anterior segment examination with +90 lens were carried out. Hemodynamic characteristics of ophthalmic artery, central retinal artery, nasal posterior ciliary artery and temporal posterior ciliary artery have been investigated by color doppler ultrasonography. Results: No statistically significant change was found in mean pulse per minute and mean blood pressure of the cases between before and 15 and 60 days after the operation. In operated eyes, intraocular pressure was found to be reduced significantly 15 and 60 days after the operation compared top re operation levels. No significant difference was observed in ophthalmic artery in terms of the values before and 15 and 60 days after the operation. In the comparison of the hemodynamic values of central artery before operation and 15 and 60 days after operation, significant increase was observed in systolic maximum velocity, end diastolic velocity and mean velocity and significant decrease in resistive index. Conclusion: Positive effect on blood flow after the reduction of intraocular pressure following glaucoma operation indicates that mechanical theory and vascular theory are compatible in the pathophysiology of primary open angle glaucoma.
