The Performance Based Supplementary Payment System Issues In The Hospitals For Chest Medicine

The aim of this proje is to compare the budgetary practice between the training and research hospital centers for the chest medicine and thoracic surgery, and to rule out the problems and the analytic suggestions of the performance based supplementary payment system. In this cross-sectional study, operating under the Ministry of Health 7 chest diseases hospitals and 4 hospitals of chest training and research hospitals, by examining the data on applications for payment of hospital performance assessed. The policlinic service was given to 28.783 patients per month at the 7 chest hospitals at which maximum number of patients are examined and maximum number of operations are performed and total of 604.461 medical examination points were obtained. The total medical examination/intervention point was found as 47 %. 32 policlinic patients and 1.310 performance points were calculated per day for each medical specialist at the second step chest hospitals. After all, the policlinic service was given to 38.962 patients at the 4 education and research hospitals and total of 818.928 medical examination point was gained on 2008 January. The ratio of total medical examination to the total venture point was found out as %36. 16 policlinic patients and 931 performance points were calculated per day for each medical specialist at the second step chest hospitals. At training and research hospitals; taking into consideration training activities and to be weighted total beds treatment services, different performance evaluation methods. should be used. A detailed performance evaluation would be useful in the investigation for both group of two hospital inpatient and outpatient treatment services
