Epidemiological and clinical studies have shown that comorbidity rates in bipolar disorder are quite high. Obsessive compulsive disorder is the most frequently encountered anxiety disorder in bipolar disorder and its frequency is reported as 9-35%. The existence of comorbidity anxiety disorder affects sociodemographic and clinical features of bipolar patients. It has been reported that the response to the treatment in these patients is low, psychotic and mixed episodes are excessive, substance use and suicidal attempts are high and hospitalization rates are higher. Although comorbiditiy rates are high in bipolar disorder in the epidemiological and clinical studies performed recently, studies on sociodemographic, clinical and response to the treatment in comorbidity conditions are limited. In order to shed light to the issue clinical features and treatment process of a case with comorbidity of bipolar disorder and obsessive compulsive disorder has been presented. On the contrary to the facts reported in the literature, the case was presented since it was found interesting that obsessive compulsive symptoms in manic period were aggressive.
