Our vesicovaginal fıstula experiences

Vesicovaginal fistula cases treated in our clinic betvveen 1983-1998 has been evaluated retrospectively. Etiologic causes and approaches to treatment have been discussed. 70.6 % of the cases were caused by gynecologic re- asons vvhereas the remaining 29.4 % were due to obstetric reasons. Three cases treated by electrocoagulation, eleven cases by transabdominal-transvesical approach and another three by transvaginal approach. After eva- luation of the results, 66.6% (2/3) success rate has been reached with electrocoagulation while this rate was 100% (3/3) at transvaginal methods and 72.7% (8/11) with transabdominal-transvesical approach. Three cases which failed at the first application of transabdominal-transvesical method ended up with recovery after the se- cond treatment with the same method. Although the number of cases are not enough to give a solid decision, we concluded that the transvaginal approach may be preferred as a first method in vesicovaginal fistula by con- sidering the lesser amount of morbidity, minimum blood loss, easiness of operation and control över the operated area, shorter hospital stay and simultaneous treatment of stress incontinance.
