Bacakta lokal ağrı, hassasiyet ve şişlikle kendini gösteren diyabetik kas infarktı, tip 1 insüline bağımlı diyabetik hastalarda nadir görülen bir komplikasyondur. Bu bulgular kliniğimize diyabetik ülser nedeniyle yattıktan beş gün sonra 72 yaşındaki erkek hastamızda görüldü. Kitle önce bir abse odağı olarak düşünülmüşken cerrahi eksplorasyonda izole ekstensor hallusis longus kas infarktı olduğu gözlendi. Nekroze kas eksize edilerek defekt alan ince kalınlıkta cilt grefti ile kapatıldı. Bu yazıda literatürde ilk kez diyabetik hastada ekstensor hallusis longus kas intaktı olgusu sunulmuştur.
