Çok uzun zincirli yağ asidi dehidrogenaz (VLCAD) eksikligi mitokondriyal yağ asidi oksidasyon bozukluğu olup otozomal resesif geçis gösterir. Tandem kütle spektrometri ile plazmada yükselmiş açil karnitinlerin artışının gösterilmesiyle VLCAD tanısına yönlenilebilir. Bu yazıda, annesinde gebelikte HELLP sendromu olan ve 15 günlükken periferik dolaşım bozukluğu, hipotoni ve izleminde metabolik asidoz, dilate kardiyomiyopati gelişen VLCAD eksikliği tanısı almış bir olgu sunulmaktadır.
